Takashi Yano bb42852062 Cygwin: pty: Implement new pseudo console support.
- In this implementation, pseudo console is created for each native
  console app. Advantages and disadvantages of this implementation
  over the previous implementation are as follows.

  Advantages:
  1) No performance degradation in pty output for cygwin process.

     sequences between real terminal and pseudo console.

  4) Redrawing screen on executing native console apps is not necessary.
  5) cygwin-console-helper is not necessary for the pseudo console
     support.
  6) The codes for pseudo console support are much simpler than that
     of the previous one.

  Disadvantages:
  1) The cygwin program which calls console API directly does not work.
  2) The apps which use console API cannot be debugged with gdb. This
     is because pseudo console is not activated since gdb uses
     CreateProcess() rather than exec(). Even with this limitation,
     attaching gdb to native apps, in which pseudo console is already
     activated, works.
  3) Typeahead key inputs are discarded while native console app is
     executed. Simirally, typeahead key inputs while cygwin app is
     executed are not inherited to native console app.
  4) Code page cannot be changed by chcp.com. Acctually, chcp works
     itself and changes code page of its own pseudo console.  However,
     since pseudo console is recreated for another process, it cannot
     inherit the code page.
  5) system_printf() does not work after stderr is closed. (Same with
     cygwin 3.0.7)
  6) Startup time of native console apps is about 3 times slower than
     previous implemenation.
  7) Pseudo console cannot be activated if it is already activated for
     another process on same pty.

Takashi Yano 83b2d576c8 Cygwin: pty: Limit API hook to the program linked with the APIs.
- API hook used for pseudo console support causes slow down.
  This patch limits API hook to only program which is linked
  with the corresponding APIs. Normal cygwin program is not
  linked with such APIs (such as WriteFile, etc...) directly,
  therefore, no slow down occurs. However, console access by
  cygwin.dll itself cannot switch the r/w pipe to pseudo console
  side. Therefore, the code to switch it forcely to pseudo
  console side is added to smallprint.cc and strace.cc.

Takashi Yano 169d65a577 Cygwin: pty: add pseudo console support.
- Support pseudo console in PTY. Pseudo console is a new feature
  in Windows 10 1809, which provides console APIs on virtual
  terminal. With this patch, native console applications can work
  in PTYs such as mintty, ssh, gnu screen or tmux.

Corinna Vinschen 2b72887ac8 Cygwin: clocks: fix a hang on pre-Windows 10 machines
when calling clocks too early in DLL init, the vtables are not correctly
set up for some reason.  Calls to init() from now() fail because the init
pointer in the vtable is NULL.

Real life example is mintty which runs into a minor problem at startup,
triggering a system_printf call.  Strace is another problem, it's called
the first time prior to any class initialization.

Workaround is to make sure that no virtual methods are called in an
early stage.  Make init() non-virtual and convert resolution() to a
virtual method instead.  Add a special non-virtual
clk_monotonic_t::strace_usecs.

While at it:

- Inline internal-only methods.

- Drop the `inited' member.  Convert period/ticks_per_sec toa union.
  Initialize period/ticks_per_sec via InterlockeExchange64.

- Fix GetTickCount64 usage.  No, it's not returning ticks but
  milliseconds since boot (unbiased).

- Fix comment indentation.

Corinna Vinschen c05df02725 Cygwin: implement extensible clock interface
- Drop hires_[nm]s clocks, rename hires.h to clock.h.

- Implement clk_t class as an extensible clock class in new file clock.cc.

- Introduce get_clock(clock_id) returning a pointer to the clk_t instance
  for clock_id.  Provide the following methods along the lines of the former
  hires classes:

	void		clk_t::nsecs (struct timespec *);
	ULONGLONG	clk_t::nsecs ();
	LONGLONG	clk_t::usecs ();
	LONGLONG	clk_t::msecs ();
	void 		clk_t::resolution (struct timespec *);

- Add CLOCK_REALTIME_COARSE, CLOCK_MONOTONIC_RAW, CLOCK_MONOTONIC_COARSE
  and CLOCK_BOOTTIME clocks.

- Allow clock_nanosleep, pthread_condattr_setclock and timer_create to use
  all new clocks (both clocks should be usable with a small tweak, though).

- Bump DLL major version to 2.12.

Mark Geisert 29af5b27cf Define internal function mythreadname() -- revised
This new function returns the name of the calling thread; works for both
 cygthreads and pthreads.  All calls to cygthread::name(/*void*/) replaced
 by calls to mythreadname(/*void*/).

Corinna Vinschen 8cff1569fc strace: Make sure strace timer isn't copied to child process
At fork time the .data and .bss segments of the Cygwin DLL are copied
over to the child process.  This also copies the strace timer since
it's in the .bss segment so far.  Fix that by moving the strace timer
out into the .data_cygwin_nocopy segment.

Corinna Vinschen 6e623e9320 Switching the Cygwin DLL to LGPLv3+, dropping commercial buyout option
Bump GPLv2+ to GPLv3+ for some files, clarify BSD 2-clause.

Everything else stays under GPLv3+.

New Linking Exception exempts resulting executables from LGPLv3 section 4.

Add CONTRIBUTORS file to keep track of licensing.

Remove 'Copyright Red Hat Inc' comments.

Corinna Vinschen 4889f730c1 Reduce stack pressure throughout Cygwin
* dcrt0.cc (initial_env): Reduce size of local path buffers to
        PATH_MAX.  Allocate debugger_command from process heap.
        (init_windows_system_directory): Very early initialize new global
        variable global_progname.
        * dll_init.cc (dll_list::alloc): Make path buffer static.  Explain why.
        (dll_list::populate_deps): Use tmp_pathbuf for local path buffer.
        * exceptions.cc (debugger_command): Convert to PWCHAR.
        (error_start_init): Allocate debugger_command and fill with wide char
        strings.  Only allocate if NULL.
        (try_to_debug): Just check if debugger_command is a NULL pointer to
        return.  Drop conversion from char to WCHAR and drop local variable
        dbg_cmd.
        * globals.cc (global_progname): New global variable to store Windows
        application path.
        * pinfo.cc (pinfo_basic::pinfo_basic): Just copy progname over from
