* include/fcntl.h (_fmode): Remove declarations and
compatibility defines.
(_setmode, setmode): Remove prototypes.
* include/stdlib (_fmode): Add declarations and
compatibility defines. Change type to int.
* include/io.h (_setmode, setmode): Add prototypes.
* samples/fmode/all.c: Adjust includes.
