	(pthread::thread_init_wrapper): Change __pthread_exit to
	thread->exit().
	(__pthread_cancel): Call method thread->cancel().
	(__pthread_exit): Remove.
	(__pthread_setcancelstate): Ditto.
	(__pthread_setcanceltype): Ditto.
	(__pthread_testcancel): Ditto.
