* crt0.c: Ditto.  Cleanup ancient cruft.  Add dummy calls to cygwin_premain*.
* include/sys/cygwin.h: Remove old stuff.  Move premain declarations nearer to
other cygwin-specific function declarations.
* globals.cc: Add comment.
