* cygheap.cc (cygheap_dummy): Rename from cygheap_at_start.

Browse Source 
(cygheap): Accommodate name change to cygheap_dummy.
(cygheap_init): Ditto.
(cygheap_fixup_in_child): Simplify slightly.
* fork.cc (fork): Add an advisory comment.
